Anyone know what bike this might be from? It appears to be a case reed configuration.

That's what I thought, but i never knew they had a case reed intake design. There is no intake on cylinder itself, only exhaust ports.

Well, there ya go! I had no idea. Clearly I am not up on anything remotely close to new or 125cc. haha.

It's a beautiful design. Takes only an hour or so to do a full top end. You can even leave the head on and still pull the jug - no problem.

Yammie 125s were case reed starting in 1988. The "5ny" code is for 2002 YZ125.
